Below is a list of the most frequently used Special Collections and Archives Department forms, along with a description of each form's purpose. Follow the link to the form you wish to see. Each SCA on-line form was designed to be printed from your local system. They are provided for your convenience, to be filled out prior to your visit to the Cline Library Special Collections and Archives Department. - Deed of Gift Agreement
- is the standard gift instrument used for donating materials to the Cline Library Special Collections and Archives. For more information on donating to SCA, see Contributing to the Collections.
- Use Fee Schedule
- is a list of fees charged for the commercial use of Special Collections and Archives materials.
- Photograph Order Form
- is an order form to obtain copies of archive images. If you plan to use the archive images for the purposes of publication, reproduction, or use in an exhibit, slide show, or business display, this form must be accompanied by the Application for Permission to Publish/Produce.
- Application for Permission to Publish/Produce
- is a contract with Special Collections for the use of SCA images in a publication, slide show, exhibit, advertisement, or business display. This form is accompanied by a "Conditions of Use" statement.
- Copying/Reproduction Prices
- is a price list of SCA reproduction services, including black-and-white and color prints and slides, Polaroid copies, audio and video tape duplication, and digital imaging.
