Proxy Account Information

Faculty and Staff of Northern Arizona University may wish to authorize proxy representatives (graduate assistants, office staff) to check out both Cline Library and Interlibrary loan materials on their behalf by applying for a Cline Library Proxy account. By completing the application below the main account holder assumes full responsibility for all materials checked out on their account according to Library Circulation Policy.

The account holder may designate more than one proxy. Materials charged on the proxy account will receive the usual faculty or staff loan period. All correspondence will be sent to the original account holder.
