Community Borrower Application

This form is for Arizona residents to obtain borrower's privileges at the Cline Library. You must be 18 years old to apply for this card. After filling out the form, please come to the front desk on the first floor to pick up your card. You must provide picture identification with a current Arizona address to qualify for this card.
